IT Readiness Form
The IT Readiness form has been updated to be more understandable and easier to collect information that ensures accessibility and the appropriate stewardship of MSU data. The information is required to assess the initial acquisition and/or renewal of any IT-related product or service.
Without this form, necessary reviews cannot be completed, and the purchase will not move forward. Please be sure to sign and include the IT Accessibility Acknowledgement.
Required Attachment for IT Procurement Reviews
After you complete the IT Readiness form, please download and attach it to your KFS requisition or provide it to your MSU Procurement agent. You will also be emailed a copy of your answers.
These questions align with the MSU Institutional Data Policy and MSU Web Accessibility Policy.
